What insurance does the practice accept?
We do not accept any insurances. This practice is cash pay only. We can provide you with a claim form to submit to your insurance for out of network benefits. Please ask for a claim form at each appointment.
What is the cost of services?
Initial appointments are an hour long and cost $300. Follow up appointments are generally 20-30 minutes long and cost $150.
How Do I Schedule My Initial Appointment?
Go to the Initial Appointments page. Read the scheduling guidelines carefully then use the calendar to select your appointment slots.
Why was my initial appointment declined and what should I do next?
Generally initial appointments are declined when they are not scheduled following the guidelines on the page. There is no staff to schedule appointments for you. When you are able, please go back to the initial appointments page and follow the guidelines to schedule.
Are all appointments virtual?
Most follow up appointments are able to be virtual. However, most initial appointments will be in person. Please schedule your initial appointment with the plan to be in person. I will confirm whether we will be virtual or in person prior to your appointment.
How Do I Log On for my Appointment?
To log on for your appointment go to the Services tab and click “Start Virtual Appointment” Please be timely. Your timeliness can affect many others downstream.
