Initial Appointment

  IMPORTANT!

READ THESE GUIDELINES BEFORE REQUESTING YOUR INITIAL APPOINTMENT: 

· All initial appointments require an hour.  

. To secure an hour, please select 2 consecutive open slots, as each slot is 30 minutes 

eg November 18th 3pm & 3:30pm.

· You will receive an email from Patient Fusion confirming your appointment once it has been approved.  This is the only confirmation needed.  

Please be sure to check your spam folder.

· You will receive an email requesting you to "check in" 1 week before your appointment. This is required as we no longer utilize paper forms. Please complete all requested information as thoroughly as possible.

· If you need to cancel your appointment, we request that you do so by calling the office at 470-419-5859 with a minimum of 24 hours notice.
