Medication Management / Follow Up Appointment

•  Medication Management appointments should be scheduled for one 30 min slot unless otherwise discussed.

• If you need to cancel an appointment simply leave a message on the office phone at 470-419-5859.   A minimum of 24 hours’ notice is required in order to avoid the missed appointment fee.

• If you need to reschedule an appointment simply submit your new request using  the section below and put in the note section that you are rescheduling.  Again a minimum of 24 hours notice is requested to avoid the missed appointment fee.
