Click link below to pay for general medication management appointments or any appointment 30 minutes or less in duration.  Also used for no show of follow up appointments ie for any missed follow up appointment or follow up appointments cancelled/ rescheduled with less than 24 hours notice. 

Click link below for a 30 day refill of your non controlled medication.  

For refill of controlled medication outside of appointment times eg Stimulants, Benzodiazepines. Limited to a 30 day supply.  Appointment necessary for any further refills.

For completion of documentation discussed and agreed upon.
